Who is eligible to apply for a grant?

Parents/guardians and professionals involved in their care may apply on behalf of a child or young adult (under 21) with disabilities who resides within an 80-mile radius of Saratoga Springs. Successful applications will include documentation of diagnosis from a medical provider, as well as a letter of medical justification in support of the equipment or service requested.

How do you apply for a grant?

How are grants awarded?

Grants are awarded based on need and level of funds available at the time of review. Our Medical Advisory Panel and Board meet regularly to review, discuss, and award grants in a timely manner. Once a grant is approved, our Executive Director will contact the family and organize the allocation.

Please note that we recognize that a grant application requests sensitive medical & financial information. We pledge this information will be held in the strictest of confidence.

What are some examples of a typical grant?

Adaptive equipment including:

  • Bicycles and tricycles
  • Strollers
  • All-terrain wheelchairs
  • Stationary play equipment
  • Electronic devices with communication software
  • Camp tuition
  • Approved therapies
  • And more!

Our list is ever-growing, because we recognize that everyone’s obstacles are different, and so are the ways they navigate them. Items requested in grants may vary, but must have the support of a medical professional that relates to barriers imposed by the child’s diagnosis.

  1. Gather the following required documents. Once received, The MAP will begin review of the application:
    1. Insurance denial of request from child’s insurance (if applicable)
    2. Letter of need from medical professional (MD, PT, OT, SLP, etc.)
    3. Specific information about the grant (cost, brand & where to purchase, size and specifications as applicable)
    4. Download Individual Application
    5. If applicable, Download Equipment Supplemental Form or Download Technology Supplemental Form

Local 501(c)3 nonprofits who serve children or young adults (under 21) with disabilities and is located within an 80-mile radius of Saratoga Springs. Successful applications will demonstrate inclusiveness, community, and the ability to demonstrate the impact of the award requested.

How are grants awarded?

Grants are awarded based on need and level of funds available at the time of review. Community grants will be accepted from July 1st – December 31st, and again from February 1st – May 1st annually. Grants submitted outside of those periods will be considered during the next grant window. Decisions will be finalized at the conclusion of each grant window. Requests for more information and decision on applications will be submitted via email.
